Commonwealth Community Care is the only primary care practice in Massachusetts with 30 years of experience providing healthcare informed by and designed for people with disabilities.
We’ve learned that to provide excellent care, we need to take time, listen, and respect the self-knowledge of our patients. Our practitioners are trained in the special healthcare needs of people with disabilities and are committed to the principles of consumer direction, independence, and self-determination.
Depending on your needs and preferences, we provide care in the home or in our specially equipped, accessible offices. We are available 24/7 and coordinate care as needed with your preferred services and providers and with the people in your life.
Our holistic approach means you have access to a multidisciplinary team of healthcare professionals working in close collaboration. Our primary care physicians, nurse practitioners, physician assistants, social workers, physical/occupational therapists, and behavioral health specialists coordinate care based on each patient’s specific needs. Our administrative staff provides assistance with transportation and referral arrangements, and our Durable Medical Equipment (DME) specialists manage patients’ equipment and supply needs and expedite their delivery.
Commonwealth Community Care has offices in Boston, Framingham, Lawrence and Springfield and provides care for people in nine Massachusetts counties.
